First Time Office Visit


While we make every effort to stay on schedule, the nature of our specialty requires that we accommodate many emergencies. When emergencies occur, we will try to minimize your delay or contact you to make alternative arrangements. We ask your understanding in these situations, recognizing that we will extend emergency service to you should the need ever arise.

Please arrive 15 minutes early for your appointment to allow enough time to complete the necessary insurance and patient registration forms. Also, please bring the following items:

1. A list of your current medications, including dosage and frequency.
2. Your past medical records, including any test results, studies or films.
3. The name, address, and telephone number of any out of town physicians who may have provided care for you in the past.
4. Your insurance card(s) and social security number.
5. Any insurance authorization referral if required by your insurance company.


Please dress comfortably so that you're prepared for any test your cardiologist may order. Warm-ups or sweatsuits are both appropriate and practical. New patients can expect to be at our office approximately 1-2 hours, although times may vary. We'll try to let you know in advance if we anticipate your visit to be longer. At the end of your examination, your physician will explain his findings to you. Again, if you have any questions, don't hesitate to ask.
